Apply for Funding

Dear Community Member:

The 98.3 LNH Children�s Auction has been blessed with considerable growth over the years and with it, a greater responsibility to distribute funding in a way that will benefit our community with maximum impact. Thus, our application process has changed.

There are now four categories of application to choose from. Process A, B, C and D. Below is a mission statement for each process, pick which one best describes your needs and submit your application no later than Thursday, October 31.

Category A

Mission Statement:
To efficiently fund local non-profit organizations that provide basic needs to area children. This will include but not limited to, food, clothing, shelter, holiday gifts, and hygienic needs.
1) Organization Name and Address
2) Contact Person Name, Phone and Email
3) How many �units� do you deliver per year? If food or clothing based, please include tonnage or individuals served.
4) What is your cost per unit and total budget?
5) What amount of funding are you requesting and what will it be used for?
6) Who is on your Board of Directors, and who is your director/manager?

Please send all requested information in an email with the appropriate attachments to apply@childrensauction.com. Submissions must be received no later than Thu., Oct. 31, 2013 for consideration.

Category B

Mission Statement:
To efficiently fund those non-profit organizations that work to eliminate or mitigate the need for basic and extended services for children.
1) Organization Name and Address
2) Contact Person Name, Phone and Email
3) Please attach latest 990 form and Audited Financial Report
4) Attach proof of Insurance Policies, Employee Training Manuals, Annual Audit, Board of Director list and coverage area
5) Amount of funding requested
6) On a separate sheet please provide the following:
a) A one page description of your organization, your mission and the program for which you are applying for funding
b) A brief explanation of your cost per unit, versus service impact
c) Logic Model: Show how your mission and outcome
alignment match the Children�s Auction mission as stated above
d) What outcomes are measurable?
e) What indicators demonstrate that outcome?

Please send all requested information in an email with the appropriate attachments to apply@childrensauction.com. Submissions must be received no later than Thu., Oct. 31, 2013 for consideration.

In addition to the requested information, we reserve the right to schedule an on-site interview with your Executive Director and Board Chair/President. We may also select two board members for phone interviews. You will receive notification of the site visit date and time by November 15th.

Category C

Mission Statement:
To efficiently fund those capital initiatives that improves the lives of children or enhance the ability of agencies to deliver their services.
1) Organization Name and Address
2) Contact Person Name, Phone and Email
3) Please attach latest 990 form and Audited Financial Report
4) Attach proof of Insurance Policies, Employee Training Manuals, Annual Audit, Board of Director list and coverage area
5) Amount of funding requested
6) On a separate sheet please provide the following:
a) A one page description of your organization, your mission, and the capital program you are applying for funding
b) A brief explanation of your cost per unit, versus service impact
c) Logic Model: Show how your mission and outcome alignment match the Children�s Auction mission as stated above

Please send all requested information in an email with the appropriate attachments to apply@childrensauction.com. Submissions must be received no later than Thu., Oct. 31, 2013 for consideration.

To fund those agencies that have an Educational/Recreational aspect to provide a service to children in the Lakes Region in which the child may not be able to afford otherwise.

2. On a separate sheet please provide the following:
a) A one page description of your organization and your mission
b) A brief explanation of your cost per unit versus service impact
c) Show how your mission and outcome alignment match the Children�s Auction mission as stated above
d) What outcomes are measurable?
e) What indicators demonstrate that outcome?
3. Who is on your Board of Directors, and who is your director/manager?
